Decision log (ADRs)

Architecture decisions for the cartridge concept, one page per decision. An open question (index.adoc, OQ list) that gets resolved lands here with its reasoning — it never just vanishes from the list.

ADR-001 — No in-module model-generation picker
ADR-002 — A C ABI, pull-based, is the runtime contract
